The office is located in Middleburg, Florida with in person appointments available Monday through Thursday 8:00am to 4:30pm and Friday mornings. Secure telehealth visits are also offered to clients throughout Florida, Georgia and New York.

Dr. Perrier is in network with many major plans including Aetna and BlueCross BlueShield, Cigna and UnitedHealthcare, Medicare and others. For out of network coverage, superbills are provided for reimbursement and payment is due at the start of each appointment.
